| | | When the petition gets approved, the employer or agent is sent Form I-797, Notice of Approval and a copy of it is forwarded to an American Consulate. The RN should submit the original, photocopy, or facsimile copy of the Form I-797A or B together with the following: |

| • | A passport or other travel document valid for at least six months beyond the period of stay in the U.S. and with at least one blank page |  |
