For a permanent resident to apply for a Declaration of Intention to become a U.S. citizen. This form is not required for naturalization, but may be required by some states if you wish to conduct certain business with that State.

Note on Filing Fee:

  • Checks should be made payable to U.S. Department of Homeland Security.
  • The Check or the Money Order must be drawn on a Bank or other financial institution located in the United States and must be payable in the US currency.
  • Spell out U.S. Department of Homeland Security; do not use the initials “USDHS” or “DHS”
