Which I-94 do I need to submit at the port-of-exit? I would like to return to the USA on H4 status.

(1)Entered US on H4 visa and recieved I-94 at port-of-entry(First I-94)
(2)Spouse changed employer got a new I-94 for H1B and my H4 too.(Second I-94)
(3)My H1B recently was apporved, I have never worked.(Third I-94)

I would like to leave the US for a couple of months and return back on H4. I have valid H4 stamp on my passport.
